Dr. Vahid Fallahati Discusses Hemorrhagic Cystitis After Stem Cell Transplantation

Dr. Vahid Fallahati, pediatric hematologist-oncologist and hematopoietic stem cell transplantation fellow, presented an overview of the risk factors, prevention, diagnosis, and treatment of hemorrhagic cystitis following hematopoietic stem cell transplantation at the 38th Pediatric Diseases Congress and the 24th Pediatric Nursing Congress.

During his presentation, Dr. Fallahati discussed hemorrhagic cystitis as one of the potential complications that may occur following hematopoietic stem cell transplantation. The condition can present with symptoms including blood in the urine, pain or burning during urination, increased urinary frequency, and lower abdominal pain.

Dr. Fallahati noted that the severity of the condition can vary considerably among patients, ranging from mild hematuria to severe bleeding accompanied by blood clot formation. Accurate clinical assessment, together with blood and urine tests and other diagnostic evaluations when necessary, plays an important role in identifying the underlying cause and determining the most appropriate course of treatment.

He also emphasized the importance of preventive measures in reducing the risk and severity of hemorrhagic cystitis. Adequate hydration, maintaining sufficient urine output, and the use of protective medications such as MESNA in patients receiving certain chemotherapy regimens are among the measures that can contribute to prevention. Close monitoring following transplantation can also support early detection and timely intervention.

According to Dr. Fallahati, treatment should be tailored to the cause and severity of the condition. Supportive measures, including increased fluid intake, may be sufficient in mild cases, while more severe cases may require further interventions to control bleeding, prevent urinary tract obstruction, and manage possible underlying infections.

Concluding his presentation, Dr. Fallahati underscored the importance of early diagnosis, continuous clinical monitoring, and coordinated multidisciplinary care in the effective management of hemorrhagic cystitis in patients undergoing hematopoietic stem cell transplantation.
